- Your name, mailing address, phone number and email.

- The exact web address (URL) of the material on our site.

3. What Happens Next
When we receive a complete notice, we remove or disable access to the material and, where we can, tell the person who posted it. Incomplete notices may not be acted on.
4. Counter-Notice
If your material was removed and you believe it was a mistake, you may send a counter-notice to the same email with your contact details, the removed material and its former location, a statement under penalty of perjury that it was removed by mistake, your consent to the jurisdiction of the federal court for your district, and your signature. We may restore the material within 10 to 14 business days unless the complaining party tells us it has filed a court action.
